Edit: Vampire The Masquerade: Bloodlines, when you find out who the cab driver is, and the haunted hotel.
Multiplayer would have to be CTF Jedi Knight 2. Flag carrier was going chased by entire enermy team, and I was carrying a force field generator. If I placed it too soon, it would shut down briefly to let him through, and several of the enermy would slip through. Too late and it would, well, be too late.
Perfectly timed drop, and they all ran full belt into the field, and we won the round.
Playing the marine first time in AvP. First level; just up the elevator past the airlock.
I had just come up the elevator, my motion tracker pinged, and there's a hissing in the distance, so I throw some flares around, still dark up ahead, so I put on my goggles, see the broken steam vent and observe the coridor, just waiting for something to come around the corner and scream towards me.
I wait for a while, but nothing happens, so I calm down continue forward slowly, and then an alien pops out of the ceiling above me, so I run forward, and suddenly there's 3 aliens. Shooting wildly (...with a joystick <!--emo&:p--><img src='http://www.unknownworlds.com/forums/html/emoticons/tounge.gif' border='0' style='vertical-align:middle' alt='tounge.gif' /><!--endemo--> ), I try to return to the elevator, but I'm butchered well before I got there.
...Didn't play it again for awhile, Predator is most fun anyway. Eventually I got to the awesome Orbital station, which is a really cool level. Especially if you spawn some marine bots and pretend to go around as a team. And then the (only predator with assorted weapons, since you can't spawn one with any plasma weapons) is in the station. Plus the face huggers...they'd shock you too.
FF7 also has a few good moments, along with racing across the top of the Pillar of Autumn in Halo (far better ending than Halo 2...), the twist in KotOR (which is the only time I've become evil in a game based on actual decisions I'd make (if I were that limited...)), basically every boss from Ninja Gaiden (Xbox) - and the lava level; despite how annoying it was, it looked awesome (and turned out to be one of the easier bosses), Stalingrad assault in CoD (loses its appeal quick, though).
...I'm sure there's tons more, but they've slipped my mind at the moment.

Unaccountably, there are no images of the acoustic Legendary Theme level on the internet, which seems rather odd, as it's the greatest gaming moment ever. Incidentally, my greatest gamming moment ever was dinner last Thursday. Premo-gamm!
The acoustic Legendary Theme level is perfect. There's absolutely no room for improvement. Compare this to Ico (whose end sequence is a very close contender) - the shadow creatures are seemingly created for the exclusive purpose of desecrating a masterpiece.
The <a href='http://thelbsc.co.uk/music/Legendary%20Theme%20(Acoustic).mp3' target='_blank'>song's</a> perfect, and the pacing of the levels couldn't be better. After 5 worlds of hard-rock action, you're treated to a level featuring an idyllic tropical sunset, no 'defence' segments (less strenuous that way), and a lovely boy-girl-guitar under the palm tree setting. There's no real challenge, unless you're utterly rubbish, and the tempo's a fraction of what you've seen previously. We're treated to this level half-way through the game, and as there's no real challenge (unless you're hopeless), it's the modern-day bonus level of our gaming forefathers. When was the last time we had one of those? I'm thinking Space Harrier.
But impossibly, despite the ease of the level and the mellow backdrop, you're spurred on to play just as hard as you would on normal levels (and more so if you prefer this song to the others, which you should). Every failed riff means a gap in the melody, means the girl shuffling away from you, means this slice of paradise being marred. You can't let that happen.
Naturally, it's something you've got to experience to understand.
Edit - erm, the game is Gitaroo Man. You play the song with the analog stick and a button. It's unique and delicious to the core.
many different genre's that it's hard to pick a favourite.
But one of my most memorable was taking part in a organised
attack in PlanetSide. There was hundreds of people involved
(no A.I and bots remember, its <i>all</i> human players) and
everyone was at special waypoints.
For example the tanks were at Waypoint A, Air Calvalry at Waypoint B,
Air Support at C, Infantry at D, Snipers at E, and so on.
When the time came to strike it was awesome, I was on the very edge
of my seat as I piloted a Galaxy dropship (I had 12-15 people in
there) over our target as gave the orders for them to "hot-drop".
(think Final Fantasy: The Spirits Within - that style of exiting a ship!)
And when they were out I had made another pass to let the gunners
out of gun positions (one on either side of the ship and on on the nose)
to join the battle.
I managed to do 2/3 full-ship runs before being shot down and then
joining in as a Sniper.
I loved every second and it's moments like that which make PlanetSide
an awesome game.... even if those moments are rather rare <!--emo&:(--><img src='http://www.unknownworlds.com/forums/html/emoticons/sad-fix.gif' border='0' style='vertical-align:middle' alt='sad-fix.gif' /><!--endemo-->

I'm going to have to add "using Omnislash at the very very end of Final Fantasy VII".
Weather you have leveled up to it or not, you still got access to it and your Limit guage flew up so fast. It was superb.

Single Player : Deus Ex. Magie Chow's appartment. She just seemed suspicous first time I met her... So I shot her in the face. Stupid maid set off the alarm, and MJ12 troops started pooring out of the walls. I didn't expect that.
I popped maybe 2 guys and made a mad dash for the capture point. In the midst of capping, 2 tanks push for where I was capping. I stood up and their barrels aim for me, when suddenly they explode.
3 friendly tanks rolled into their base and blasted them allowing me to cap.
Best 2 hours ever.
The scariest experience was in NS, ns_origin. I was one lone Marine, running away from an Onos. He almost devoured me a few times, but couldn't quite hit me. I eventually jumped down into the vent right by Xenoform Research, and the Onos couldn't get to me. Great moment!

Deep-frying the tentacle monster with the engine booster.

In the city level, a pair of hunters burst through a door into a courtyard you're defending. They come strolling through the clouds of dust they kicked up when they battered down the door, the sunlight glinting off their armor - it just blew my mind.
Boarding the prophet's personal carrier in Halo 2 and making with the smackdown was great as well.
The ending duel in KOTOR no matter what side you chose. That was awesome.
Okay, remember the Rogue Spear demo? It was in a museum and there was this one exhibit that was like a tiny building with 2 doors on either side and it was just a box really. Me and another 2 guys were in there getting ready to move out an suddenly there are like 5 people all around us. We duck into the box, they start shooting. Everyone is peeking around corners and trying not to get shot. That was pretty awesome.
Whenever I'm playing The Specialists and I get into "the zone" and kill 5-10 people until I run out of ammo.
Toppling an AT-AT in Star Wars: Battlefront. Especially awesome since it's a joint effort between the pilot and the gunner.
Destroying the Invincible in X-Wing.
Going into the part of the train station with the super high roof and the light streaming through in Half-Life 2.
Capping the flag in Starsiege: Tribes a split second before you get killed by like 4 people at once.
Riding on a speeder around Star Wars: Galaxies and finding hidden ruins that aren't on any of the maps.
Pandora Tomorrow: the 2nd to last level in the TV station, at the end. That was awesome.
Skydiving without a parachute in NOLF.

I think all of WoW has amazed me. I cant wait until I get to behold Oxynia.
