HELP! NS is trying to load a map mid-match

I am having a problem with NS lately and it is making it so I cannot play for more than five minutes at a time. It is rather frustrating and I cannot find answers anywhere. Over at 187 they told me to reinstall, but that didnt work. I'm hoping one of you might be able to shed some light on the situation. heres my problem:
The game starts normally, and I begin to play and after a few minutes, seemingly at random and without prompt, my client will attempt to load t0a0.HL1
I have no idea what this is, but I spawn looking down through a grate, in what looks like a HL map (see below).
Then the client disconnects and returns me to the 'find servers' dialog. I have no idea what is happening, but I took a condump and here are the last few lines;

this happens to me on ANY server and there is nothing I can do to stop it. Anyone have any ideas? Not being able to play ns sucks anus <img src="style_emoticons/<#EMO_DIR#>/tiny.gif" style="vertical-align:middle" emoid="::onos::" border="0" alt="tiny.gif" />

    Dont try to cheat...

    Edit: Well, maybe I am a little bit unfair, cause I dont know the exact circumstances.

    So here goes the all will be well link:

    Follow my installation guide and you will be able to play NS. (Albeit, without any hack-support, but meh...)

    that -hook happens after it kicks me from the game, it has nothing to do with a cheat.
    on 187, the server I usually play, +hook is enabled and legal for gorges to get around better, I think it's part of the amx mod they use. But even when I go rine this happens.
    Av!rix on the 187 forum told me about the -hook commands
    "The hook thing does not matter it happens to everyone. Im looking up that error message atm."

    As for your guide, That is pretty much exactly how I installed ns, with the exception that I did not have to rename the folder because it automatically used my steam ID. I can get in and play normally, and have been able to for some time. All of a sudden this started happening. I have no idea what to do.

    Did you uninstall NS first?

    specifically, remove all of your CFGs? (just delete the damn directory)

    It sounds like you got a screwed up CFG trying to do somethign silly.

    t0a0a IS (iirc) the HL hazard course.

    Either that or you have a 3rd party program doing something silly.

    The first console line gives it away:

    "Loading game from SAVE\quick.sav..."

    You've got a key bound to quickload.
