Most important is the minimap_extents entity, scaled to enclose the map on a square base. Then you can then render the minimap and the .hmp file which will be used by the game to define the extent of the commander view. At first you will only need one commander_camera entity ( I only use one ) to define the height of the commander camera, if there are areas in which you want to change the commander height you can place more setting the height for different areas.

Yup. I've got a few errors with it since it changed a bit so someone with knowledge on that would be better off explaining how to generate the minimap correctly. There's some info on it in the guildlines.
