I started the game today (since update gave me new hopes) and even though my FPS was good (60-65), it felt like it was 15 FPS. I believe this is called stuttering, and now the game is unplayable.
A lot of this is server based. Type net_stats into the console and check the server tickrate, if it's lower than 30 that's probably what is causing your problems. I have been experiencing this too but I haven't had a chance to play it on a decent server with less than 24 people on it.

This is true, but it's also true that overlays are absolutely causing big problems with framerate. Any kind of messenger overlays, mumble overlays, fraps, xsplit, basically anything that tries to overlay over NS2 (or even behind it in fullscreen) seems to cause a framerate problem. It shouldn't affect the built-in Steam overlay, though.
There is a client stuttering issue with D3D9 that is NOT either of the above mentioned issues that is being worked on as we speak.
Also confirmed that FPS sometimes lowers for no reason for a period of time randomly, then goes back up.
This sounds a lot like a known issue with OpenGL that is currently being looked into. Are you using OpenGL by chance?
Also, the build 254 hotfix just went live a few minutes ago, so see if you still have the issue after updating.
I am having both of the problem of micro stuttering whilst using DX9/11, and the problem above. The random FPS drops were a consistent problem before 252/253, and were happening every 30-60 minutes.
If I use OpenGL then both problems disappear, however the game crashes at least once a round.
In it's current state it takes the patience of a saint to actually play the game. This isn't good enough, it's just disappointing.
I also question the logic of launching another free weekend straight after a new build release with so many bugs. If i was a new player I would instantly be turned off to the game in its current state.
On the plus side, Biodome is amazing and visually the best map you've produced yet. It's just a shame I can't play it.

Well, that's not what I was thinking of, but if you head over to technical support we/someone can hopefully try to work through the issue with you.
This is also why we originally were going to push only the rendering changes on Monday so we'd have "enough" (lol) time to fix most of the issues involved before releasing Reinforced and going into the free weekend/PAX. Things ended up a bit messier than that, but I think we're still relatively on track.

Just chiming in, I started playing around 8pm PDT and with the latest patch on a UWE East server I had no more random stutter or huge FPS dips like I did in 253.
