60 fps when spawning. Constant 29.9-30 fps when playing. All setting low/off

Doesn't matter what res/settings I played I can't get more than 30 fps in game, though I get 60 fps when I'm spawning in, which is weird.
Tried about every fix I could find before posting here.
Formatted my PC a week ago, fresh and up-to-date drivers on just about everything.

Edit: Also bear in mind your CPU speed is low for an I7, mobile i7 chips are pretty slow compared to their desktop counterparts.
Try doing an r_stats while slow in-game and take a screenshot and post it here too.
Seems D3D9Device::update is grabbing most of it. What was the reason of that one again, I forgot.
As I remember ingame you had both cpu and gpu cap, lets see what will produce the best results..
Mess around with both physicsMultithreading and gpu-acceleration on/off and see if that produces any difference.
The gtx 680m should be able to perform better then 30fps considering its all set to low. I did a quick compare to my old 570 which performs similar in many tests but which ran better fps in ns2. Well.. perhaps 30fps lategame.
I could not see your memory usage but that may be a added effect. (my windows atm using 2.42GB due to AV, windows itself, browsers etc.. Start a game and it can easily hit 4GG, which is more then your rig has.)
CPU many may consider 'low' for ns2 but I wonder how it will do now ns2 is becoming more and more multithreaded.
Use a program like HWmonitor and prime95 and torture test your cpu a bit. See if it doesnt become to warm. You want to keep a eye on both CPU usage % and cpu frequency %. Prime SHOULD make both. If not > heating issue.
Is NS2 supposed to run better in fullscreen? I've got an external monitor setup and like to run games in windowed mode so I'd like to know if that was the issue or not

NS2 will always run better in FullScreen due to how Windows works. If you can, always go FullScreen without being in any window.
That's odd, I only see about 1-2 frames difference between fullscreen and fullscreen-windowed (out of ~90-120 usually)
Going from 60 fps to 30 fps is happening because you have Vsync enabled in your graphics options - its a typical symptom of Vsync for any game.
You will have Vsync applied anytime not in fullscreen, and you can still have Vsync on when in fullscreen.
So I suggest disabling that, not only will you maintain whatever FPS your CPU is capable of, but you will also incur less input delay.
