Gates also tend to stop people from clearing lanes...There go your naturals, oops gorge tunnel up outside of base! Pub games skulls tend to bite RTs early and get distracted midway-late game.
Meds? Well on larger servers it becomes pretty clear very fast on where/who should be medicated.
Personally, I tend to not med based on wanting my marines to die. Having 7 rines with lmg push a hive that has a decent fade, a gorge, and their entire team spawning.... mean while a different tech point just got taken and they are most likely about to drop a second/third hive.
