My understanding is that whole way of talking grew out of crackers and wanna-be crackers trying to avoid FBI IRC chatrooms scanners. They scan for certain words, like hacker.... thus we have things like h4X0r.
Of course, the script kiddies started using it to be cool, and of course they game as well, so now it's part of our culture as well. It's a bit annoying at times, but it is an interesting example of the flexibility of language.

I already knew a lot of the stuff people are posting here, and know WHAT 1337, just don't know how it came about. I seemed to remember it came from quake.
l33t (or 1337 or whatever...) originated from a mispelling of the word "elite". Check www.dictionary.com if you don't know what "elite" means. Much the same way "pwnd" came about... mispelling of "owned". Ownage, owning, and pwning all orignated from "pimp" wannabes. "Joo are MY **** biotch, I OWNZ JOO NOW!" And all that kind of annoying crap. And "joo" didn't come from a mispelling of "you". It's origin is derogatory and I won't even mention it. (Although, it is accepted now as "you" most of the time and not the other form) I won't even start into the origin of "myG0t". You can guess.
I read that in an article somewhere. Yes, it's all derived from an incredibly immature and rude community. Mostly teenagers. I even knew 18-20 year olds that were "in" to it, back in the day. I personally think it'll die out soon, as people that use 1337 5P34K now are instantly branded as "n00bs", all out geeks or are just joking around a bit. It means nothing anymore, while a couple of years ago you were a l337 h4x0|2 for using it.
It orginated in the quake communtity, but I'd say the CS community is what really magnified it to what it is today. Even early, like beta 6, I remember signing onto msn and my friends that played cs all had indecipherable names and would send me jibberish messages that a cryptologist would have a hard time with. (Back then, I never played FPS's, just starcraft and nice simple 2d games my comp could handle... I got to hear all about it though... now those fools... bwa ha ha... I have the l33test comp of the bunch! They can barely run NS while I get 120 fps BWA HA A HA HA HA... err... okay right.)

As with most things, its actually probably a combination of things.
For l33t speak. Yes it is the old warez to-avoid-cops talk originally, but also the Quake community added another "depth" to it if you will, making it as much their own as the warezies and hax0rs.
And as for llamas, while I've always thought the term originated (also from the quake community) from lamers. I've never heard an explaination for it like Thansal's, but it seems equally likely. So its probably both.
