unless i'm mistaken, an entity is anything on a map that is not specifically a wall. Weldable things are entities, res portals are entities, closed vents are entities, electric bolts are entities, as well as ladders, moving doors, and the like.
Just wanted to update you really quickly with a recent development. I've made a big performance optimization, such that all spawn entities are tracked independently of HL, and are deleted when the map loads. This means there are now 96 less entities on all NS maps! With other improvements from the mappers and some other code changes, this brings some maps below the 200 range (!), and most of the other maps below 300. This should make a HUGE difference in server performance in 1.1 and beyond.

Yes. But you still shouldn't be doing other stuff at the side of running a server, at least not on the same comp. EDIT: Says me, the guy who never hosts a dedicated server... But I still think that^ would be a bad idea.
With the new version .. I could definatly see an p4 2.0 or amd xp2000 hosting two 16 peep servers no problem. (with the chance of slight lag on heavy maps like hera when played for a long time). I have seen my server host a 26 person 1.1 game on a heavy map (amd xp2000) with almost no lag (expect from those ppl in like Turkey!). Flay has done a great job of getting the cpu useage down.
