Thor's 250mm cannons stun Kerrigan. Get two at each choke; win. I was actually quite surprised at how easy it was on hard.

PF walls are also fun and hilarious, but she can lift and one shot them sometimes you need to have a backup orbital read to plop down.

As for the worms... just make a lot of siege tanks, and point some of them into your base using the high ground near the thing. Get a huge control group of banshees for the ones outside your base - most of the time there's no detection en route or even at the worm. It's really a ton easier than defending against huge amounts of mutas and brood lords (traditional anti-Terran siegebreakers...).

I finished my playthrough on Brutal yesterday and I must say I found that defending against air @ brutal was easier than defending against worms @ normal (probably because I didn't even think about using psi emitters back in normal, but still). The armory upgrades offer a lot in the anti-air department, especially turrets AoE, turrets life, Vikings AoE, Vikings range 11. Range 11! That's 2 more than broodlords iirc. I only had to restart once using this layout:

4-5 vikings behind the mineral line
    2 turrets / bunkers / tanks + 1 psi emitter + 5-6 vikings per choke
6-8 turrets + rest of viking production for the artifact
20-30 marines to deal with Kerrigan and guard artifact.

Almost never had to micro my vikings to kill broodlords; the superior range and the AI took care of it.

btw there's a bonus mission. In the mission where that guy steals a thor and takes it into the capital city of the dominion, if you take your thor to the bottom right and kill some civilian building a "secret document" will drop and trigger a mission.
