Once again, I'm sure the devs are aware of this, but I figured I'd post it anyway.
Sometimes when a phase gate is placed too close to the walls and aliens swarm it, phasing through automatically puts you in the wall, or else you fall through the world.
Then you just fall and fall and fall.
