Attaching an image generates the wrong code

When I attach a file, there's a convenient (albeit broken) link that appears under it that adds the code into the post to show the picture... it doesn't work though. Should just be , but it's something alse ATM.
It should also surround the image in spoiler tags... but that's just my preference.

I know, I know, but I have to do that manually by using the button to show me the bad code, snipping out the url from that, deleting it, THEN adding all the code back in.
That window doesn't come up when you click the image button?
Alright, example time! I have a small image I'll be using that's pretty much the same as my profile pic.
1) click "choose file"
2) choose the damn file.
3) Roll over the thumbnail. Oh look at that! There's a very convenient button there that says "insert image". I think I'll do that!

Huh? That doesn't look right! I'd better fix it.

5) Hit spolier tags, then image tags, and paste the url. There! That's much better. Now why couldn't that "insert image" button have done it properly?
Haha no, I think I was just explaining it badly.
Just suggesting alternate ways.
If I can't pronounce it, or the name scares me -- in this case both -- I won't use it.
