Beacon bug in Mineshaft

A player on my team complained of being beaconed, as exo, right into the waterfalls in cave, killing him. How common is this experience?

    I already spawned (not beaconed thought) in a command chair in a custom map; so I believe it is map specific. It might also be that the exo was moving and when beaconed "walked" into the waterfall and fell (which is really possible). If the latter is the case, maybe ensuring the player look at the obs or command station might help. (just brainstorming)

    I've had it plenty of times where everybody else teleports except me. I have no clue what causes that...

    Not only this, but we had a guy beacon to a completely different tech point. Whole team beaconed to Shipping and 1 lone marine beaconed to ET. lol!
