Diving knife disappears until reloading save.

I've noticed after using my diving knife for a long period of time (fighting, cutting creep weed etc.) It will disappear from my characters hand/inventory completely without an "broken" message or anything. However after saving and reloading the knife will reappear in my characters inventory (but not on the quick bar.)
