check the entity info_mapinfo . There you can set the commander view height. I dont know if numbers below 0 are possible . but you can try it. In case they arent possible just select your whole map (remember to have texture lock on) and shift it a bit up.
The height set in info_mapinfo is actually the Z-coordinate of the height of the commander. So any value between +/- 4096 should work. Not that there's not much left to see at -4000, and I wouldn't get too close to +4096.
And the location of the entity is irrelevant. Just don't put it in the void. And don't use more then one. All of this information (and a whole lot more) can be found in the <a href='http://www.natural-selection.org/Mapping_Guidelines.html' target='_blank'>mapping guidelines</a>.
