1. Darkness - Yes, most underrated form of scarying players. If they see what they are up against, it isn't as scary.
2. Powerlessness - This can be achieved quite easily by throwing in monsters you can't kill, or too many monsters to take down. The only alternative is to run away. Makes for good gameplay too since some people have no concept of this in games.
3. Unknown - This sort of plays in with darkness a little. If you don't know what you're fighting, it can tap into the scariest fear of all.. the fear of the unknown.
4. Realism - I doubt there will ever be a game truly scary on SNES or NES systems. It's simply because, you never forget it's not real. The more real it is, the more you feel like you are there. So the more real it is, the easier it is to put yourself in that position.
5. Surprise - Tell me you didn't **** in your pants with a flashlight in one hand and a shotgun in the other crawling through those dark dark vents in half-life only to find a headcrab leap at you, and I'll admit I'm wrong. Surprise, in combination with an eerie atmosphere and abience, you get people jumping out of their seats.

Some of the people that played Clock Tower on the SNES have apparently been fairly scared.
The whole point of it of course was being a small girl that had to run away from a serial killer. Of course you can't fight back so....
