Sometime the draw distance in game is smaller than your LOF making your enemies disappear.
Usually not a problem. But I have found more and more places where it have become a problem. Even on a small map such as Summit. Although more noticeable on the bigger maps such as Refinery.
Am I the only one noticing this? And is there a way to 'fix' it?

I don't think that the maps should be changed. As they have been as they are for a while. And are good in there current state. So unless the mapmakers can change the draw distance of the enemies per room. Then I don't see what they can do.
They can't change it per room but for the map overall. Have a look at the old ns2_goliath thread for details.
The issue is increasing the LOS per player leads directly to a loose of frames per second for any situation where the new entity update distance kicks in (the game code has to check a larger area for entities, animation updates, physics etc.).
So overall ns2 mappers learned to focus on generating "close room" combat while creating the illusion of "wide rooms". Did you never wonder what all those trees do in kodiak, or the half open doors in tram etc. :P
It's weird design decission though, that the map itself is not culled by distance and the players are. I guess, that even the few players removed from the update queue improve performance....
Pretty much what I thought. :-/
