There is almost no way in HI to see where shots are coming from unless you get really good with the HL damage tracker thingy but still that isn't that great for finding who shot you.
I mean even Rainbow Six is better than HI as far as interface goes. As far as realism in games goes I think its probably better for there to be some interface rather than none. Rainbow Six still manages to get the "Oh dear I'm going to die" theory without having no way to look at your health (Your character limps and is slow) which is something HI lacks.
And to be honest HI won't get any decent penetration (And by this I'm talking "regular" players) until it does something to be user friendly. The interface should be designed to allow people to glance and say "Oh I am prone" and the important stuff that needs to be determined quickly. Of course this can be done without an interface....
