Well, I can bug my chemistry teacher on how to get rid of that black stuff, but for now I think Comet would do best.
Failing that, a strong alkali-based cleaner should do....a REALLY strong one. Just be sure to wear rubber gloves, or your hands might begin to hurt really badly.

Hard water is just water with lots of minerals dissolved in it. This will only cause white calcification buildup in your shower head. It won't cause black scum between tiles.

How about some piping hot concentrated sodium/potassium hydroxide(desolving it in water is an exothermic reaction so just make it concentrated enough and it will be really hot)?
Leave it there for a long enough on skin and it denaturates the collagen into gellatine and turns the fats into the sodium salt of the fatty acid(AKA soap) as well as glycerol.
I really wouldn't recommend hydrochloric acid as it evaporates readily(HCL is formed when it exits solution) which will make stuff corrode really well, if you get some on your skin that slimy feel is probably a tiny amount of fat on your skin being turned into soap, it's not a problem as long as you don't let it just sit there. Any acid that doesn't evaporate and is reasonably strong will be perfect for removing the mineral gunk but I don't think it will do anything at all to the mold/dirt, sure vinegar smells nice, but if you need something a little more powerfull buy a bag of lactic/citric/other acid from your super market, you can usually find them in the spice section(I think they are intended for making marmelade and removing calcium salts from coffee machines and stuff).
Sodium hydroxide will only eat a way a little at metals that it is directly in touch with, especially aluminum. You need to wash it away with lots and lots of water afterwards. If you have to touch it for any extended periods of time use rubber gloves.
If chemicals don't work or are too impractical(if you have to use liquid flourine to get rid of it you are probably better of with mechanical scrubbing of some kind <!--emo&:p--><img src='http://www.unknownworlds.com/forums/html/emoticons/tounge.gif' border='0' style='vertical-align:middle' alt='tounge.gif' /><!--endemo-->).

Step 2) Spray bleach on comet.
Step 3) Cover generously with a layer of Scrubbing Bubbles.
Step 4) Let sit for a day, with the door closed.(Make sure noone's in the bathroom when you do this.)
Step 5) After obtaining gas mask, grab a hose. Open door, and from outside, open fire(water).
NOTE: Don't enter bathroom for about a day, you might die. (The chlorine fumes are dangerous, or so I'm told.)
