The scariest villain ever is the Grand High Witch from the film The Witches (The film of the Roald Dahl book of the same name). She turns kids into mice with sweets and then kills them. This film is meant to be aimed at children, but it scares the pance off me even now. Look:

What makes a good villian?
If you say that it's the embodiment of pure malovelence, I'd agree with everyone who suggested Darth Vader. Vader is the perfect dark knight, a character which I found less fearsome than fascinating (until Lucas got the <i>great</i> idea of doing the sequels and spoiled it all...).
If a villain has to represent the petty, mean aspects of mankind, I'd vote for Dougray Scott in his role as Sean Ambrose in MI2, although most John Woo - villians would come close. The character is traced by greed, selfishness, and narcism - you don't fear him, either, but you despise him.
As for genuinely insane villians, I'll agree with those who suggested Gary Oldmans role in Leon.
On the subject of scary villians, I'd definetely vote for the conceptually perfect killer - the Xenomorph.
Now, for videogame villians.
My personal favorites, in no particular order, are Magus from Chrono Trigger, although one could argue whether he's truly a bad guy, Simons from Deus Ex, who beats Agent Smith hands-down on his own turf, Jon Irenicus from Baldurs Gate 2, who's just one of the aesthetically coolest mages in any visual medium, and, worse and more hated than all others combined, the HL multiplayer cheater.
Something tells me that everyone assuming the G-Man to be a bad guy will experience a few surprises in HL2. At least, that's what I'd do if I was the writer.
villains... well theres so many, but here's my list anyhow
1. sonny forelli in the GTA movie (yes they're making 1, and yes, i did see parts) 2. al pachino as satan in the devil's advocate, while he is evil, u do just love his style 3. blofeld... well, he is meant to be the evil head honcho of "spectre" 4. agent smith... good suit 5. stalin... he ruined communism for every1, now nobody will see the bueaty of divine sharing
Well, Sarevok (Baldur's Gate) comes to mind, though you could argue that he only turned out the way he did because he was a victim of circumstance (If you've played Throne of Bhaal you know what I'm talking about).
And, as Nem so thankfully pointed out, Irenicus. Though again you could argue that he turned out the way he did due to circumstance as well (although stealing peoples souls is just plain evil).
Nichole Thorne from Max Payne. Absolutely no remorse about having Max's wife and baby murdered.
That really big one eyed monster alien end-game boss from Duke Nukem 3D. Popping up into the middle of a stadium only to come face to face with THAT creature is still one of my fondest gaming moments.
The Covenant from Halo. Their <b>religious</b> mission in life is to exterminate all humanity.
The manager of channel 6 from UHF. I can't think of his name at the moment, but I'm sure someone will remind me before I remember... Funniest movie evar!
Kefka from Final Fantasy 6. I don't know how they managed to evoke so much feeling with that low quality 16-bit laugh. He was such a great villain, because you loved to hate him, and when you got to the end, it was so satisfying to finally bring him down.

Durandal, Marathon. He is so...creepy and evil but also good.
Max Payne, Max Payne. While he's not really the villian, he is bad. I mean, he murders people!
Taiidani Empire, Homeworld. Gee, does violating a forgotten treaty really deserve an Atmospheric Deprivation weapon?
The Beast, Homeworld: Cataclysm. It's the Zerg! In Space!
Kharaa, Natural Selection. Anything trying to wipe out humanity is a villian. That's why I love the Kharaa.
Sydney from vagrant story owns them all... allthough hes not a true villain.
